This form is to be used to report any incident of inappropriate behavior or harassment experienced by students involved with the speech and theater program at Downers Grove South High School. Include as much or as little detail as you are comfortable with, but keep in mind that the more detail you include, the more helpful the document may be in the future. If you don’t remember something exactly, or can’t recall certain details, that’s okay. Write down what you remember.
Once you've completed the form, the following steps will be taken:
1. The DGS Drama sponsors will receive notification that a form as been filled out. We will follow up with you shortly.
2. You are more than welcome to report the incident to a sponsor, coach or director in person as well, along with Ms. Martinez, Student Activities Director.
3. If you are in need of immediate help, please seek out a counselor ASAP.
